Rates: $155.00 for initial session and $140 for follow up sessions. I do work on a sliding scale on a limited basis for those without insurance.
Hours:
My hours vary by the week. I am usually pretty flexible and have both daytime and evening appointments. I do not typically work on the weekends but can occasionally fit in a Saturday appointment if necessary.
Services may be covered in part or in full by your insurance carrier. I am currently contracted with Blue Cross Blue Shield of CT, United Healthcare, Optum Behavioral Health, Aetna, Husky, MHN/Tricare, APS Healthcare and Medicaid. I can work with other insurance carriers as an out of network provider, but it is your responsibility to find out about and understand your out of network coverage before scheduling your first appointment.
Here are some questions you may want to ask when contacting your insurance carrier:
Do I have mental health coverage?
Do I have a deductible and how much of it has been met?
How many visits per calendar year are covered?
Do I need prior authorization?
Do I need a referral from my primary care physician?
Is this provider in net work or out of network?
What are the out of network benefits?
It is expected that prior to your first appointment you have contacted your insurance provider and understand your benefits. If you still have questions, please contact me and I'd be happy to help.
Payments:
I accept cash, checks and all major credit cards. You are responsible for any returned check fees incurred.
Cancellation Policy:
I understand that sometimes unavoidable circumstances can keep you from attending your appointments. If you are unable to make a scheduled appointment, I ask that you give me as much notice as possible so that I can rearrange my work day. I have set aside that time for you specificaly and it is often difficult to fill in appointments without several days notice. Cancellations without 24 hours prior notice or missed appointments will be charged a $50 cancellation fee which is not covered by insurance. Exceptions to this policy are dangerous weather and illness/medical emergencies.
